Use this checklist for site observations

Record the property context another technician will need on a return visit. This is a general documentation checklist, not a treatment plan or jurisdiction-specific pesticide record. Use your required forms and approved procedures for applications, products and regulated reporting.

Before the visit: confirm the property and access

Start by reviewing the previous site record and identifying anything that needs confirmation.

  • Property name, address, building or unit and visit date.
  • Authorized site contact and agreed inspection areas.
  • Access arrangements, occupied areas, pets and restricted spaces.
  • Previous reported activity and any unresolved access limitation.

During the walkthrough: locate each observation

Use consistent room or zone names so another technician can revisit the same place. Separate customer reports from your own observations.

  • Record the exact area, what was observed and the observation date.
  • Note reported activity separately with the source of the report.
  • Record visible site conditions relevant to the inspection, without guessing a cause.
  • Photograph the area and a useful detail where permitted.
  • Identify inaccessible areas and the reason they were not inspected.

Before leaving: make follow-up clear

Summarize the observations with the authorized contact. Record what still needs review, who is responsible and how that person was notified. Keep treatment documentation in the required application record; a free-text site note does not replace it.

  • Changes to standing access or preparation instructions.
  • Unresolved findings and the person notified.
  • References to required treatment or inspection records held in the appropriate system.
  • Preparation details for the returning technician.

Illustrative observation

“September 15, ground-floor stockroom: customer reported activity behind shelving. Rear corner inaccessible because of stored cartons. Photograph shows the blocked area. Site contact will arrange access before the next inspection.” This records evidence and a limitation without inventing a pest identification or treatment instruction.
